1) )  I/We have gone through and understood the terms and conditions of admission / website into class as published in the admission notice and I/we hereby declare that all the information provided in this registration form is true, complete and correct in all respects. 

2) I/We understand that in the event of any information being found false, incorrect or incomplete at any stage, the candidature of my/our ward, for admission in Dayawati Modi Academy, Modipuram Meerut, is liable to be cancelled by the Management of the School.

3)I/We declare that my/our ward is medically fit in all respects and that in the event of him/her becoming medically unfit, I/we shall immediately inform the School authorities. I/We shall not hold the School authorities responsible should anything happen to our ward as a result of his/her medical condition. I/We hereby agree that should our ward be admitted into this School, I/we have no objection to send my/our ward to School or elsewhere on School related assignments even on non-school days, if his/her participation is considered mandatory by the School authorities.

4) I/We know that the registration fee is non-refundable and registration is not binding for admission. Admission is subject to suitable vacancy & child's performance in the qualifying test as per school norms. I/we further agree to abide by all Rules and Regulations of the School as framed/amended by the Management of the school. 

5)Aadhar Card of your ward is mandatory at the time of Admission.
